Dear parent,

Please fill up the form overleaf for entering the details of your child in CCE report card. Also send two copies of colour photographs( Size 3cm x 2.5cm including border, in grey uniform) of your child, for affixing on the report card. Submit the form duly filled in, along with photograph to the respective class teacher on or before 03 October 2010.
